If you are under regular review by Dr. Kelly and want a repeat prescription of treatments, please complete the following form. Please also arrange for any recent blood tests to be faxed to 01-2920510 (you will need to contact your GP). It is essential that we have access to these before issuing a prescription for your safety.
There is a €20 charge to cover the cost of processing repeat prescriptions. The invoice and details of how to settle will be emailed to you.
